The immune system during sepsis

Septic shock is the most severe outcome associated with pathogen infection in the bloodstream. It is a life-threatening condition invariably leading to multiple organ dysfunctions. Currently, septic shock is one of the most frequent causes of death in intensive care units worldwide.

However, it is already known that sepsis-induced multiple organ dysfunction is not a direct effect of the pathogen invasion itself but rather an overreaction of the host immune system against the infection. Many strategies aiming at holding back the extreme response of the immune system have been developed but little progress has been made.

A group of researchers from Santa Catarina, Rio Grande do Sul and So Paulo, in Brazil, have shown that blocking the receptor of bombesin/gastrin-releasing peptide (GRP), a peptide involved with the activation of neutrophil and macrophage immune cells, improves survival in sepsis animal models. Working with RC-3095, an antagonist of the GRP receptor developed by Nobel Laureate Dr Andrew Schally, the group showed that this molecule attenuates the release of the host's exacerbating immune response elements, and reveals a new inflammatory pathway and potential target for new drugs. The study was part of a research effort to investigate additional functions and potential clinical applications for the GRP receptor, initiated by Drs Gilberto Schwartsmann and Rafael Roesler at the Federal University of Rio Grande do Sul.

Led by Drs Felipe Dal-Pizzol and Fabricia Petronilho at the Universidade do Extremo Sul Catarinense, in Brazil, the group has recently teamed up with Drs Andrew Schally and Norman L. Block at the University of Miami Miller School of Medicine to investigate the potential link between GRP and TLR-4, a receptor found in neutrophils and macrophages. TLR-4 is one of the molecules responsible for spreading the word that the body has been invaded by microbes and that something must be done quickly.

In a paper entitled "Gastrin-releasing peptide receptor antagonism induces protection from lethal sepsis: involvement of toll-like receptor 4 signaling" and published in Molecular Medicine, the group shows that RC-3095 reduces the levels of circulating TLR-4 and other immune elements associated with an extreme inflammatory response. Additionally, the group has found that patients with septic shock have greater amounts of GRP if compared to those with less severe forms of sepsis. These higher levels of GRP may explain why septic shock patients develop multiple organ dysfunctions and present the highest mortality rate among sepsis patients.

The study also shows that in animal models, administration of RC-3095 limits the spread of infection beyond the abdominal cavity, indicating the potential of RC-3095 in preventing the complete breakdown of the host's system.

Besides its clinical relevance, the study provides new insights on the roles of key players involved in the complex communication network triggering extreme inflammatory responses. Together, these findings may open new avenues for the development of effective therapeutic strategies to treat sepsis and related inflammatory diseases.

Also statements of tax returns for the previous five years, as well as your spouse's financial information so that you have a good handle on your situation when you start to interview bankruptcy lawyers. Finding an experienced, vetted, and personal bankruptcy attorney is a make or break deal to your future financial situation. Look for an attorney who will provide personal services, insist on personal meet instead of meeting an associate or assistant.

In terms of fees, don?t bargain for the cheapest services lest you may be hiring someone who either lacks in services or is of a poor reputation. At the same time, unless you are dealing with a tortuous bankruptcy case that involves multiple parties and millions of dollars worth of assets and secured interest at stake, there is no reason to pay for one of the top-tier lawyers.

Ephron knew that the most pointed weapon of all is humour, from her treatise on the attraction and incompatibility of the sexes in When Harry Met Sally, which gave us the glamour of Manhattan but the dating horrors, sexual tensions and frustrations of modern life everywhere. The hairdos and jackets have dated since 1989 but the repartee burns as bright as ever and the Katz deli orgasm scene is still a show-stopper.

Who cares if Meg Ryan?s Sally Albright was a journalist who seemed to spend more time at lunch with her mate Carrie Fisher than working? Or that Billy Crystal?s droll Harry was a political consultant with no discernible interest in politics? Ephron?s ability to combine crackling one-liners about exes? new girlfriends ? ?What?s she like?? ?Thin, pretty, big tits. Your basic nightmare? ? with the search for love, not just sex, in the city, lifted her into the pantheon of great cinema writers.

?I interviewed Ephron when You?ve got Mail came out and was struck by how seriously she took her feminism, for all her lightness of touch. She had, she told me, come out of a generation in which girls like her went to smart colleges but were not expected to achieve much in their careers.

Looking around the room decades later at a group of successful baby-boomers, she noted that they were ?the world?s best?dressed, oppressed group?. Yet in Silkwood, whose script bagged an Oscar, she broke out of a metropolitan-chic milieu to make a film about environmental conditions in blue-collar America which was way ahead of its time.

That mixture of the serious and playful inspired me as a young writer. Observations on female physical insecurities, like the essay I Feel Bad about My Neck, were triumphs of psychology and observation. Ephron truly believed men and women could be happier if they shared more of their uncertainties.

One of the sub-texts of the Harry-Sally relationship is that they have to learn to stop speaking in the quips and statements of their types: Beatrice and Benedict on the Upper East Side. Her movie Heartburn was a tear-jerker about faithlessness in marriage yet uplifting and hopeful.

?There was an old-school, spontaneous New York kindness in Nora too. Interviewing her at the Dorchester, I produced a messy nose-bleed just as we sat down on the silk chairs. The PR gazed in horror and insisted that I leave. Nora was having none of it, ministered to me herself ? and insisted that we still had our allocated time. Later, I got a note suggesting I expense the dry-cleaning bill as ?nose-bleed, brought on by over-talkative American?.

What a miss she will be. Her words made us busy, vain and fearful men and women on both sides of the Atlantic feel a little better about ourselves. No small thing, that.

Eat This Poem is a new blog that combines food and poetry in much the same way Feasting on Art considers food and art. Eat This Poem is written by Nicole Gulotta who has an MFA in poetry enabling her to marry beautiful words with delicious food. Working with her on a collaboration only felt natural and when she suggested a post investigating William Carlos Williams and Pieter Bruegel, I knew I was fated to work with her.

While an undergrad I double majored in both English and History of Art and in my final semester, I did an entire course in William Carlos Williams. My major paper considered the collection of poems titled "Pictures from Bruegel" which is a composite representation of Bruegel?s work through the viewpoint of Williams. The poetry illustrates the way in which Williams? eye follows the canvas and his impressions while gazing upon the works.

Pieter Bruegel the Elder, the 16th-century Flemish Renaissance painter, was a master landscape artist. Within these landscapes, small narratives are depicted which are focused on the life of the working class society. The scenes are split between work and play with subjects varying from wedding dances to haymaking as pictured in "The Harvesters." This panel is one of six created to depict the seasons and represents the late summer/early autumn when peasants would reap the hay.

Rather than focus on the action of the peasants, Bruegel chose to take a broad view of the landscape to portray the way in which the land is transformed through farming. Although the landscape is the heart of Bruegel?s painting, the subjects of "The Corn Harvest" by William Carlos Williams are the peasants in the foreground of the canvas. Grilled Corncobs with Parmesan, Lime & Paprika
Inspired by a dish served at Ms.G?s in Potts Point

Yield: 2 servings

2 corncobs
1 tablespoon butter
1/2 cup grated parmesan cheese
juice of 1/2 of a lime
1/2 teaspoon hot paprika
handful of fresh coriander leaves

Husk the corncobs and remove all of the silk. Rub them with a bit of butter and place on a grill or in a griddle pan over medium-high heat. Cook for about 5 to 10 minutes depending on the size of the corncob, rotating every couple of minutes until the cob is evenly cooked. The corn will be soft and tender when done.

Remove from heat and immediately cover with a quarter cup of grated Parmesan cheese so the residual heat in the corn melts the cheese. Squeeze over the lime, dust with the hot paprika and scatter the coriander leaves. Serve immediately.
